【重要】2020年度健康診断日程のお知らせ

学生の皆様へ

当初、4~5月に実施予定だった健康診断は、「新型コロナウイルス感染症緊急事態宣言」を受け、感染拡大予防の観点から夏以降の実施で調整中です。
日程および詳細につきましては、政府発出の緊急事態宣言解除後、感染リスクを判断した上で学内メールにてお知らせいたします。

※「健康診断結果通知」は夏以降の実施日より約1か月を要します。
※「健康診断書」は別途手続きが必要となり、さらに1週間後の発行となります。

なお、就職試験等で健康診断書が早急に必要な場合は、 健康管理室(kenkou@slcn.ac.jp) にご相談ください。
内容によって最寄りの医療機関で個別(自己負担)にご対応いただく場合もあります。

学生の実習・インターンシップ・就職試験等に関わる医療機関・諸施設 ご担当者様へのお願い

「新型コロナウイルス感染症緊急事態宣言」を受け、4~5月に実施予定だった健康診断は夏以降に延期することとなりました。
つきましては当該年度実施の健康診断による「健康診断結果通知」「健康診断書」発行が、夏以降の実施から1ヶ月以上を要しますことをご理解いただきますようお願い申し上げます。
なお、貴施設が健康状態の確認を必要とする場合、提出期限を延長していただくか、前年度の健康診断結果の利用を可としていただく等のご配慮をお願い申し上げます。

[IMPORTANT] 2020 Academic Year – Notice about Health Checkup Dates

To all Students,

In light of the novel coronavirus infection state of emergency, the health checkups originally planned to be conducted from April to May are currently being arranged for after summer to prevent the spread of infection.
Following the lifting of the government-ordered state of emergency, we will notify you via campus email of dates and details upon determination of the infection risk.

*The “Health Check Results Notice” will require one month after post-summer checkup dates.
*The “Attending Physician’s Statement” will require a separate process and an additional week for processing.

Should you immediately require your Medical Report for employment exams, etc., please contact the Health Management Office (kenkou@slcn.ac.jp).
You may be asked to get checked individually at your nearest medical institution (at your own expense) depending on the situation.

Request to those in charge – Medical Institutions and Various Facilities Involved in Student Practicum, Internships, Employment Exams, etc.

Because of this, issuance of the “Health Check Results Notice” and “Attending Physician’s Statement” for this year’s health checkups will require one month after your post-summer appointments. Thank you for your kind understanding.
I hereby request your consideration by either extending the submission deadline or allowing the use of last year’s health checkup results when your facility requires confirmation of one’s health condition.
